Connect for Health patient handouts address each of the primary, evidence-based behavioral goals for weight management. They can be shared with patients and used as a tool to guide weight-related conversations with patients and their families. Provision of physical activity and nutrition educational materials meet the HEDIS criteria for counseling for nutrition and physical activity.
